Early Registration - before 1/16/10                        
Individual $350    Attending with spouse $325

From 1/16/10 to 3/22/10
Individual $375    Attending with spouse $350

Late Registration - 3/23/10 to 4/12/10
Individual $395    Attending with spouse $370

Each participant - including spouses attending together - must complete a separate registration form.

Registration closes April 13, 2010
$75 of the registration fee will be retained for registrations cancelled after March 22, 2010.

A block of rooms is being held at the Quality Inn Airport Hotel until April 5, 2010, at a rate of $69.00 per night. Transportation to/from Phoenix Sky Harbor Airport and the Symposium location is included. To make your reservation, call Quality Inn at 480-969-3000.

To get the special $69 rate, you must reference Frontiers' Member Care Symposium.

If you are interested in sharing a room, please contact Quality Inn at (480) 969-3000 and ask for Donna Heitkotter, Group Sales Manager, who will make every effort to accommodate your request.

You are free to make reservations at another hotel and to make your own arrangements for transportation to/from the airport and Symposium location if you prefer.
